Port Document Required: Porting Form (Switzerland)

To port a Switzerland number to AVOXI, our supplier requires a completed porting form.

Why This Is Required

Every Switzerland Toll-Free number has a registered user/owner on file with the Federal Office of Communications (OFCOM/BAKOM), which oversees media and telecommunications in Switzerland. The porting form must be completed by that registered owner — not just anyone requesting the port.

What You Need to Provide

  • The completed Porting Form (Switzerland).docx
  • The entire form filled out, with the required boxes checked depending on what you're requesting
  • Must be signed by the registered owner of record with OFCOM/BAKOM — if you're not the owner of record, the actual registered owner must complete the form instead

 

If you're unsure who the registered owner is:

  • Contact your current carrier — they're the only party authorized to disclose the registered owner, update the owner of record, or provide the necessary authorization
  • You can also try checking one of these Swiss government portals, though they're external and not always available (you may see a 404 error, or the link may change):

 

How to Submit It

  • Log in to the Genius Number Portal and go to the relevant number's profile page.
  • Upload the completed form directly within the platform.
  • AVOXI's number team reviews it — the in-application status updates automatically once reviewed.
