Why This Is Required
Your current (losing) carrier needs written authorization from someone with authority on that account before it will release the number for porting. This protects against unauthorized number transfers and is a standard requirement across most porting carriers.

Must be completed by a legal representative from your company

Upload the completed form directly within your AVOXI platform.
Once uploaded, the document is automatically submitted to AVOXI's team for review — status updates in real time within the platform.
If a document is rejected, you'll receive a notification with specific feedback so you can correct and resubmit it.
