Why This Is Required
Latvia's carrier requires a signed Letter of Authority from an authorized contact on the current (losing) account before it will release the number for porting. This confirms you have the authority to move the number to AVOXI.

Must be completed by an authorized contact on your current (losing) carrier's account — an unauthorized signer can delay the port
Must be completed by a legal representative of your company
