What You Need to Provide
- Official Document Name: 【MKY N型雲端總機語音服務】使用申請書 (“Taiwan Application Form”).
- Language Requirement: All fields must be completed in Chinese (except for foreign names, which may remain in their original language).

Mandatory Dual Company Seals: The form must be stamped with two distinct seals:
- Your Taiwanese Company Seal
- Your Legal Representative's Seal
Activation Constraints: Application forms that are missing required seals or not completed in Chinese cannot be registered, and the requested number will not be activated.
