The porting process could be delayed if an authorized contact on the loosing carrier account does not complete to above form.
If your number is currently under a reseller (not a local carrier from Italy), please include the Italy carrier that owns these numbers (also known as the upstream/local or Donor Operator), as this information is required to submit the request.
