What You Need to Provide

• Your customer details

• A local South African address

• The details of the order placed with AVOXI

• The reason for acquiring the number

• The intended use behind the number

• All fields highlighted in blue in the template must be filled in

• Signature from an authorized individual

• The date the document was signed

2. Upload it in your AVOXI online account.
