To port your numbers to AVOXI, you need a completed and signed AVOXI Letter of Authority (LOA).
Why This Is Required
Your current service provider needs a signed LOA before it will release your numbers. The LOA proves that your company approves the move to AVOXI.
What You Need to Provide
- The completed AVOXI Letter of Authority
- The date of your request
- Your company name
- The name of your current service provider
- Your full service address. Include the city, state, and zip or postal code.
- Your Billing Telephone Number (BTN). This is the main number on your account with your current service provider.
- Every phone number you want to port
- The signature of an authorized representative of your company
- The printed name of the person who signs
- The job title of the person who signs
- The date the form was signed
- Your company stamp
- Your LOA must include every detail listed above.
How to Submit It
- Download the AVOXI Letter of Authority. Fill in every detail listed above. Have an authorized representative sign, date, and stamp it.
