Port Document Required - ITU number- allocation form

For a UIFN (Universal International Freephone Number) port, AVOXI needs an ITU number allocation form.

Why This Is Required

The ITU (International Telecommunication Union) is the central registration authority for Issuer Identifier Numbers (IIN), under Major Industry Identifier (MII) 89, used for telecommunications. An IIN identifies a telecom company operating internationally. This ITU number and form is attached to your UIFN number and must be provided by your upstream carrier.
