This form confirms that your German local number is registered to an address in the same city as the number itself. German regulators require this before AVOXI can activate the number on your account.
Why This Is Required
In-country regulations for Germany require local numbers to be tied to a physical address within the same city as the number's area code. AVOXI collects this documentation to stay compliant with that rule.
What You Need to Provide
A completed and signed AVOXI Germany Local Registration Form (AVOXI_Germany Local Registration Form.docx)
Signed by the end-user of the number
Address must be located within the same city as the number ordered
Not accepted: an address outside the number's city — without an in-city address, AVOXI cannot supply the number ordered.
How to Submit It
If you provided an in-city address at order time, AVOXI will use that address automatically.
If not, log in to the AVOXI Genius platform, go to the number's order profile page, and download the AVOXI_Germany Local Registration Form.docx
Have the end-user complete and sign the form with the in-city address.
Upload the completed form from the number profile page.
AVOXI's number team reviews it — status updates automatically once reviewed. Typical turnaround: 24 hours. Urgent cases: +1 (770) 937-9735.
Note: The number will not work and remains blocked until the documentation is received and registered with the local provider.
