Documentation Required - Affidavit of Use of Number (Taiwan)

As per country requirements, an 'Affidavit of Use of Number' must be presented when ordering or registering a Taiwan number. The purpose of an 'Affidavit of Use of Number' is to describe the reason for acquiring the number, its intended use, and to provide details of the end user and authorization.

 

Please fill out a letter with the following content:

  • Company name
  • Number of employees ( could include the staff in related subsidiaries who use the numbers)
  • The authorized contact details of the person who signed the form should be the same as those provided on the passport/ID.  (Contact info person, email address, phone number, physical address)
  • The service description for the number
  • Title and name of authorized signatory, e.g., CEO, CFO  (should be the same person you are providing the passport/ID.)
  • Signature of authorized signatory (should be the same person you are providing the passport/ID.)
  • Company stamp, if available

Samples: 

  • Fill in the Yellow and Red highlighted fields—completely replace each field with your own information. If a field is not highlighted, don’t change it.
  • Have the documents signed by an authorized signatory of the Sender.
