To port a Peru number to AVOXI, our carrier requires a completed portability form, hand-signed and fingerprinted by the authorized contact/legal representative listed in the Peru Power of Attorney (Vigencia de poder).

Why This Is Required

Peru's carrier requires both a hand-signature and a fingerprint from the same legal representative named in your Power of Attorney before it will release the number for porting. Missing either one will delay or block the port.

What You Need to Provide

• The completed Peru Portability Form — must be submitted in the original Spanish version (an English translation image is included on the source page for reference only)
• Must be completed by the authorized contact/legal representative listed in your Peru Power of Attorney (Vigencia de poder)
• Must include a hand-signature of that same legal representative
• Must include a fingerprint of that same legal representative
• The hand-signature must match the identification on file as closely as possible
• Fields highlighted in yellow on the form must be filled out and selected

Both the signature and fingerprint are mandatory — the port cannot proceed with only one.

How to Submit It

1. Download the Peru Portability Form and have the authorized contact/legal representative named in your Peru Power of Attorney complete the yellow-highlighted fields.
2. Have that same person hand-sign the form and add their fingerprint.
3. Log in to the Genius Number Portal and go to the relevant number's profile page.
4. Upload the completed form directly within the platform — learn more here.
5. AVOXI's number team reviews it — the in-application status updates automatically once reviewed.
