Port Document Required - Letter of Authority - (Mexico)

When porting a Mexican number to AVOXI, our carrier requires an application form to be filled out and signed.

Highlighted in RED are the main sections we need to be filled out:

Important note: The porting form must be signed by the individual who has the power of attorney. Therefore, kindly ensure that the porting form is signed by the person listed as the authorized representative (has/was given power of attorney) in the Power of Attorney (Mexico). Additionally, please provide a copy of the government-issued ID (Legal Representative Passport or ID) for that same individual. Kindly make sure the signature matches the one in the ID.
