The supplier also requires the Taiwanese business card of this person - the card should specify at least the Taiwan business details: Contact info person name, email address, phone number, Business physical address. 

The name on the Taiwanese business card must match the name on the Affidavit of Use of Number - Taiwan. Please note that this will be verified by the supplier. This person must be available to be contacted by the supplier
