China's supplier requires this order form to process your number activation, along with specific signing and identification requirements.

What You Need to Provide

• The completed Telco Order Form, signed and stamped with your company stamp/wet seal by the legal representative and the project manager (if different from the legal rep)
• A scanned copy of the legal representative's identification, and the project manager's identification if different from the legal representative

1. Complete the Telco Order Form and gather the required signatures, stamp, and ID copies.
