To activate your new number, the attached order form needs to be filled out.
In addition, this form needs to meet the following requirements:
- Form must be signed and stamped with company stamp/wet seal by the legal representative of the company and project manager (if different from the legal rep)
- A scanned copy of the legal representative's identification, and the project manager's identification (if the project manager is different from the legal representative), is included with the form submission
