Document Required - Letter of Intent - Philippines (PLDT) LOI

A Letter of Intent (LOI), also known as a Letter of Authority, describes the reason for acquiring your Philippines (PLDT) number and its intended use.

 

Why This Is Required

PLDT (the Philippines carrier) requires this documented statement of a number's intended business use as part of its activation process.

 

What You Need to Provide

  • The completed Letter of Intent (LOI).docx, on your company letterhead, including:
    • Company name
    • Authorized Contact details
    • The service description for the number
    • Title and name of authorized signatory, e.g., CEO, CFO
    • Signature of authorized signatory
    • Company stamp, if available 
  • Only fill in the fields highlighted in yellow — completely replace each with your own information; leave everything else unchanged
  • If you don't have your AVOXI number yet, you can remove that reference

 

How to Submit It

  • Download and complete the Letter of Intent (LOI).docx per the requirements above.
  • Email the completed document to your assigned Account Manager, or to salesteam@avoxi.com — include the company name, document type, and number purchased in the subject line.
