As of March 2024, China's supplier requires this signed Authorization Letter for all China 4008 number orders. It can be filled out in English.

- The completed Authorization Letter Template,
- Must be signed by an authorized representative of the company
- Must include the signature of the end user
- Must include the signature of AVOXI
- Must be completed in English
