Avoxi uses only Gamma to port UK Toll-Free, Local, and National numbers. Gamma uses a strict porting process so it is important to have all information handy as well as a correctly filled LOA before you can submit it for approval. Please use the specific LOA below. Key Information needed on EVERY port form
Current Provider: Complete Name and Address is required, the address needs to include zip code or postal code.
New Provider - Please leave this field blank.
Site address to register against numbers - This has to be the end user's address. This must be a UK address with a UK postal code.
This is the customer address registered with the carrier (Needs to Include the Postal code or zip code)

Customer’s Company Details - This needs to match the information in the customer's invoice exactly.
Contact Person or Authorized person - This has person has to be on file with the current carrier
Numbers to be Ported - List all numbers that the customer would like to port
Account Number (Billing Account No): An account number is a unique string of numbers and, sometimes, letters and other characters provided by your current carrier, that identifies the owner of an account.
Proof of ownership: - Last invoice of current provider listing numbers or letter from the carrier listing numbers the numbers and accepting the port out.
